UNM is helping Santa Fe CONNECT conduct a survey of new clients about their experiences with CONNECT.

Eligibility: Only first-time clients are eligible for the survey. You will know that a person is eligible if you need to create a new client profile in Unite Us.

  1. After creating a profile for a new client and conducting regular navigation, read the individual the Offer Script.
  2. If the individual agrees to take the survey, read them the Survey Script.
  3. Record information about whether the individual agreed to take the survey, and (if so) their contact information in the Navigator Log. If you enter this information, you will receive gift cards as detailed below.
  4. If the individual chose to take a paper survey, please give the individual a paper survey. They can complete the survey at your location and you can mail it, or they can complete the survey within a week of their intake navigation. They can mail the survey at any mailbox.

Please read to the client:

  • We are inviting you to take a survey to help our organization and CONNECT improve services to the community.  CONNECT is a network of organizations that helps people get access to resources they need to be healthy and thrive, like housing, food, or transportation. I am a navigator for the CONNECT network.
  • The survey is optional, and it will not affect your services in any way.
  • It will help us better serve you and other individuals in Santa Fe County.
  • The survey takes less than 15 minutes, and after you finish the survey, you will receive a $20 Walmart merchandise card to thank you for your time.  
  • In the future, you may also be invited to complete follow up surveys or interviews, and you would also get Walmart merchandise cards for completing those.
  • The current survey includes questions about your experience getting your needs met through CONNECT and a few demographic questions. 
  • You can take it now or any time within the next week.  
  • The consent document for the survey tells you more about the survey, including that it is optional, confidential, and you can stop taking it at any time.

Please read to the client:

Thank you for taking the survey!  You can choose how you access the survey.  Your choices are:

  • I can give you a paper survey and pencil that you can do now or within the next week, and mail in.
  • You can get a text on your phone with a link that will let you take the survey from your phone.
  • You can get an email message that contains a link to the survey.
    • You can take the survey from this link on any phone, tablet, or computer (for example, a computer at the library).
  • Would you like to take the paper survey, or use a link sent to your phone or email? 

DO NOT READ:

  • If paper: Give individual paper copy and pen or pencil; the individual can mail it, or you can mail it (the paper survey is self-sealing and pre-addressed to UNM, with postage paid).
  • If text to phone: Please enter the individual’s phone number in the online Navigator Log.
  • If email: Please enter the individual’s email address in the online Navigator Log.
  • Please finish filling out the Navigator Log for this client now, to receive your associated gift card.

After reading the Offer Script and Survey Script to a first-time CONNECT client, if they agreed to participate in the survey, record this information and the client’s contact information in the Navigator Log.

The Navigator Log asks you:

  • Your name
  • Whether you read the Offer Script
  • Whether you read the Survey Script
  • The individual’s name
  • Whether the individual would like the survey in paper, text or email
  • The individual’s contact information for their preferred survey format

You will receive a $5 Walmart gift card when you read the Offer Script to a new individual and a $5 Walmart gift card when you read the Survey Script to a new individual who wants to take the survey. The survey is optional for CONNECT clients.

Frequently Asked Questions

No, only clients who are new to CONNECT (e.g., not already in Unite Us) are eligible.

No. The survey is optional.

Survey respondents get a $20 Walmart merchandise card for completing the survey. Respondents who also complete a follow-up survey will receive an additional Walmart merchandise card. Some respondents may also receive a Walmart merchandise card for participating in an interview.

Yes. The UNM research team will not disclose respondents’ information to any individuals outside the study team. This means no one else at UNM, no one at CONNECT, and no one outside of those working on the survey can access the information. The respondents’ identifying information is only used to invite respondents to do a follow-up interview or survey, send them a merchandise card, or link them with other data as described in the consent at the beginning of the survey.

From February 16, 2026 to [needs update]. All individuals will be invited to do a follow-up survey later in the year, and some individuals may be invited to participate in an interview.

No. Whether the individual takes the survey does not affect the services they get from CONNECT.
